The mysql_* functions are long gone, so are each() and create_function(), curly-brace string offsets now fail, passing null to many built-in functions raises deprecation notices, and dynamic properties are deprecated as of 8.2. We run PHPCompatibility through PHP_CodeSniffer and use Rector for the repetitive changes, then go through the pages by hand, because scanners miss things.
All of this happens on a staging copy set to the target PHP version. When it passes, we switch the live server and keep the old version available as a fallback for a short while, in case something odd surfaces.
